What is the MDQ?

The Mood Disorder Questionnaire screens for a lifetime history of mania or hypomania. It has 13 symptom questions and two follow-up questions.

When is the MDQ positive?

It misses many people with bipolar II. If you are suspicious, ask about hypomania directly. Avoid starting an antidepressant alone until bipolar disorder has been considered (NICE CG185).

Frequently asked questions

What makes the MDQ positive?

7 or more symptoms, occurring together, causing a moderate or serious problem.

Does a negative MDQ rule out bipolar disorder?

No. It often misses bipolar II. Ask about hypomania if concern remains.

What should I do after a positive MDQ?

Refer for specialist assessment if bipolar disorder is suspected (NICE CG185).
